What is Hospice?

Hospice is about embracing the hope to live as fully as possible, in comfort and dignity, even in the face of advanced illness.  Hospice Care focuses on quality of life, emotional support for the entire family, in-home management of medical needs, and the advanced management of pain and symptoms.

How We Help

We focus on helping keep our patients and their families free from pain, anxiety, and stress.  Our hospice team includes nurses, certified nursing assistants, chaplains, social workers, and volunteers, all working under the direction of each patient's physician.

Some of the ways we help

  • Observation & assessment

  • Skilled Nursing Care

  • Pain & symptom management

  • Caregiver education

  • Guidance on end-of-life decisions such as:

       Advanced Directives, Do-Not-Resuscitate Orders, or funeral planning

  • Emotional and spiritual support

  • Bereavement support & grief counseling

  • Personal care

  • Assistance with activities of daily living

Who Qualifies for Hospice?

  • Those diagnosed with a life-limiting condition with a poor prognosis should the disease follow it's natural course.  

  • Those with illnesses in which curative measures are no longer effective, appropriate, or desired.

  • Families and caregivers of these loved ones.

 

It is not necessary to be homebound to receive hospice care.  

Persons with any advanced illness are eligible. 

Some common diagnosis include:

ALS . Alzheimer's . Cancer . Congestive Heart Failure .

End-Stage Liver Disease . End-Stage Renal Disease . Multiple Sclerosis . Stroke .

Parkinson's Disease . Pulmonary Disease (COPD)

 

In addition, those experiencing serious multi-symptom deterioration, frequent emergency room visits and/or hospitalizations, or discontinuing aggressive treatment may also be eligible for hospice care.

When is it Time for Hospice Care?

Generally, it is time for hospice care when doctors are offering a poor prognosis.  It is better to begin hospice care early rather than late. Many people are referred too late to fully benefit from all the services offered through hospice.

How to Begin Care with Magnolia Hospice Company

Anyone can refer a patient to hospice; patients can even refer themselves.  Eligibility for hospice services must be certified by the patient's attending physician and the hospice medical director.  If you need help with a referral, contact us and we'll guide you through the process.
